Bitter Melon for Diabetes: Dosage, Benefits and Evidence

Bitter melon is a staple of blood-sugar formulas, and the meta-analyses on it disagree with each other in an instructive way. Earlier reviews found glycaemic improvement on low-quality evidence; a 2024 meta-analysis of nine randomised trials in 414 patients found no significant effect on fasting glucose or HbA1c and concluded the metabolic effect cannot be determined from the available evidence.
How Bitter Melon Works
Charantin, polypeptide-p and vicine are the named constituents. Polypeptide-p has been described as insulin-like, and proposed mechanisms include improved peripheral glucose uptake and AMPK activation. The gap between mechanistic plausibility and trial results is wide here.
Benefits
- Fasting glucose and HbA1c: earlier meta-analyses found improvement on low to very low quality evidence; the 2024 analysis found no significant effect.
- Lipid profile: examined in a 2024 review alongside glycaemic indices, with similarly modest findings.
- Nutritional value: as a vegetable it is a reasonable food. As a supplement the case is unproven.

Trials use a wide range, roughly 500 mg to 2,000 mg/day of extract, or several grams of fruit powder, over 4-16 weeks. There is no consensus effective dose, which is itself a signal about the state of the evidence.
Side Effects
GI upset, abdominal pain and diarrhoea are common. It can cause hypoglycaemia, particularly alongside glucose-lowering medication. The seeds contain vicine and can trigger favism in people with G6PD deficiency.
Interactions
Additive with insulin and oral glucose-lowering drugs, with a real hypoglycaemia risk requiring monitoring. Avoid in pregnancy: it has abortifacient activity in animal work. People with G6PD deficiency should avoid the seeds entirely.
